If you've been trying to conceive, you've probably been told to "track your ovulation." It sounds simple enough — until you're staring at a thermometer at 5:30 a.m. wondering if 97.4°F means something different than yesterday's 97.3°F, or Googling "is this cervical mucus or just discharge" for the third time this week.
I get it. When my husband and I were trying to conceive, I had the clinical knowledge — I knew what ovulation was, what an LH surge meant, how the follicle releases the egg. But living it was different. There's a gap between knowing the science and learning what ovulation actually feels like in your body. Every cycle felt like a new puzzle.
As a nurse practitioner specializing in OB-GYN, I've helped hundreds of patients learn to read their own ovulation signals. Some women have textbook signs — the cramping, the egg-white discharge, the temperature shift. Others feel nothing at all, and that's completely normal too. This guide walks through every ovulation sign, what the research says about each one, and the practical tracking methods that actually work — so you can stop guessing and start understanding your cycle with confidence.
The most reliable signs of ovulation are changes in cervical mucus (clear, stretchy, egg-white consistency), a sustained rise in basal body temperature (about 0.5–1°F after ovulation), and a positive ovulation predictor kit showing an LH surge. Secondary signs include one-sided pelvic pain (mittelschmerz), breast tenderness, increased sex drive, and mild bloating. Your fertile window spans the 5 days before ovulation through the day of ovulation itself — with the highest chance of conception in the 2 days before the egg is released.
Ovulation is the release of a mature egg from one of your ovaries. It's the single event your entire cycle builds toward. Without it, pregnancy isn't possible.
In a textbook 28-day cycle, ovulation happens around day 14. But here's what most sources leave out: very few women have a textbook cycle. According to a 2019 study published in npj Digital Medicine that analyzed over 600,000 menstrual cycles, only about 13% of cycles are exactly 28 days long. The average cycle length ranges from 21 to 35 days, and ovulation timing varies even more than cycle length — it can shift by several days from one month to the next within the same person.
What is consistent is the second half of your cycle. After ovulation, the luteal phase (the time between ovulation and your next period) is relatively fixed at 12–14 days for most women. So if your cycle is 30 days, you're likely ovulating around day 16, not day 14. If your cycle is 26 days, ovulation may happen closer to day 12.
This matters because your fertile window (the days when intercourse can actually result in pregnancy) spans about 6 days: the 5 days before ovulation and ovulation day itself. Sperm can survive in the reproductive tract for up to 5 days, but the egg is viable for only 12 to 24 hours after release. According to the American College of Obstetricians and Gynecologists (ACOG), the highest probability of conception comes from intercourse in the 2 days before ovulation, not on ovulation day.
That's why knowing your signs of ovulation, and recognizing them before the egg is already gone, is so valuable when you're trying to conceive.
Not every woman experiences all of these. Some women notice several; others feel nothing at all. Both are normal. The key is learning which signals your body sends, so you can build a personalized picture of your cycle over time.
This is the single most reliable physical sign you can observe without any tools. Throughout your cycle, the consistency, color, and amount of your cervical mucus changes in response to rising and falling estrogen levels.
Here's the pattern:
A 2004 study in Human Reproduction found that women who tracked cervical mucus correctly identified their fertile window with about 76% accuracy — comparable to many other methods and free to do at home.
In my practice, I tell patients: if you're only going to track one thing, track your cervical mucus. It's the only sign that reliably tells you ovulation is coming rather than that it already happened. Pairing mucus observation with a simple ovulation test strip gives you the best of both worlds — a predictive sign plus a hormonal confirmation.
Mittelschmerz (German for "middle pain") is a mild ache or cramping on one side of your lower abdomen around the time of ovulation. According to the Cleveland Clinic, approximately 40% of reproductive-age women experience this, though not necessarily every cycle.
The pain typically lasts minutes to hours, though it can sometimes persist for up to 48 hours. It may switch sides month to month depending on which ovary releases the egg that cycle.
What causes it? The exact mechanism isn't fully established, but it's likely related to the follicle stretching the ovary's surface before releasing the egg, or the small amount of fluid or blood released during ovulation irritating the peritoneal lining. Either way, it's considered a normal part of the process.
A clinical note
Ovulation pain is a helpful secondary signal, but don't rely on it alone to time intercourse. By the time you feel it, the egg may already be released, and you ideally want sperm waiting in the fallopian tube before that happens, not after. Think of mittelschmerz as a confirmation that your body is doing what it should, not as a starting gun.
If your mid-cycle pain is severe, lasting several days, or accompanied by fever, nausea, or heavy bleeding, talk to your provider. That's no longer mittelschmerz. It could indicate an ovarian cyst, endometriosis, or another condition that needs evaluation.
Your basal body temperature (BBT) is your body's resting temperature, taken first thing in the morning before you get out of bed, eat, drink, or even talk.
Before ovulation, BBT typically hovers between 97.0°F and 97.5°F (36.1–36.4°C). After ovulation, the release of progesterone from the corpus luteum causes a sustained rise of about 0.5–1°F (0.3–0.5°C). This creates what's called a biphasic pattern: a lower phase before ovulation and a higher phase after. When your temperature stays elevated for three or more consecutive days, it confirms that ovulation has occurred.
Here's the limitation: BBT only confirms ovulation after the fact. It doesn't predict it in advance. By the time your temperature rises, the fertile window is closing or closed. That's why BBT works best in combination with cervical mucus tracking or OPKs — the mucus and LH tests tell you ovulation is approaching, and the temperature shift confirms it happened.
For BBT tracking to be reliable, consistency is critical. Use a basal body thermometer (they measure to two decimal places), take your temperature at the same time every morning, and log at least three full cycles before drawing conclusions. Disruptions like poor sleep, alcohol, illness, or even sleeping with your mouth open can throw off readings.
Luteinizing hormone (LH) is the hormonal trigger for ovulation. About 24 to 36 hours before the egg is released, LH surges sharply — and ovulation predictor kits (OPKs) detect this surge in your urine.
A positive OPK means the test line is as dark as or darker than the control line, indicating that your LH surge is happening. Ovulation typically follows within 12 to 36 hours. According to a review published in Human Reproduction Update, OPKs are about 98% accurate at detecting the LH surge when used correctly.
A note on PCOS
Women with polycystic ovary syndrome may have chronically elevated LH levels, which can produce misleading positive results on standard OPKs. If you have PCOS and are getting frequent or confusing positives, talk to your provider about alternative tracking methods or hormone-specific monitors.

Some women notice tender, swollen, or sore breasts around ovulation. This is driven by rising estrogen levels before the egg is released, and then progesterone afterward.
A 2025 study published in BMC Women's Health that followed healthy premenopausal women over a full year found that breast tenderness and swelling peaked around ovulation and continued through the luteal phase, with significantly higher reports in ovulatory cycles compared to anovulatory ones.
Breast tenderness is a secondary sign — useful if you consistently experience it, but not specific enough to use as your primary tracking method since many things (caffeine, hormonal shifts, even stress) can cause breast soreness unrelated to ovulation.
Research supports what many women anecdotally report: libido tends to increase around ovulation. A study in Human Reproduction documented a 24% increase in sexual activity during the most fertile days of the cycle, driven by peaks in estrogen and a small but meaningful rise in testosterone.
From an evolutionary perspective, this makes sense: your body is signaling that conditions are optimal for conception. If you notice a predictable pattern of heightened desire mid-cycle, that's a helpful data point to layer with your other observations.
Some women report bloating around ovulation, caused by fluid retention as estrogen peaks. Others notice a sharper sense of smell, changes in taste, or more vivid dreams. These are the subtlest signs, and they vary widely between individuals.
I include them here because they're real. If you're the kind of person who reliably notices a particular smell differently mid-cycle, or feels bloated specifically around days 12–16, that pattern is worth noting in your tracker even if it wouldn't show up on a clinical test.

Not feeling ovulation symptoms doesn't mean you're not ovulating. Many women ovulate regularly without ever noticing mittelschmerz, breast changes, or mucus shifts. As long as your cycles are reasonably regular (21–35 days) and you're getting a period, ovulation is very likely happening even without obvious symptoms.
That said, there are situations where the absence of ovulation (called anovulation) is a real concern:
Anovulatory cycles can be tricky because you may still bleed — it just isn't a true period triggered by ovulation and progesterone withdrawal. It's breakthrough bleeding from fluctuating estrogen, and it can mimic a regular period well enough that you wouldn't know the difference without tracking.
