πŸ”΄ That second half is a proprietary blend, and it is the thing to understand before you buy. The individual amounts are not disclosed β€” only the 854 mg total for all five ingredients together. That matters because KSM-66 ashwagandha is the best-evidenced ingredient in the bottle: a 90-day trial in 46 men with low sperm counts used 675 mg daily and reported large improvements in count, volume and motility, and an 8-week randomised trial used 300 mg twice daily. Both used 600–675 mg of standalone standardized extract. Here, 854 mg is shared between five ingredients, so ashwagandha cannot be at the studied dose unless it is nearly the entire blend. Myo-inositol makes the arithmetic harder still, since inositol trials use 2 to 4 grams. Read that blend as broad support, not as a delivery mechanism for a researched dose.

On the category as a whole, the honest picture is mixed. The 2022 Cochrane review of antioxidants for male subfertility pooled 90 studies and 10,303 men across 20 different antioxidants and found they may increase live birth rates β€” an odds ratio of 1.43 β€” but rated that finding very low-certainty and concluded the overall evidence is inconclusive. And there is a limitation almost nobody mentions: the men in those trials were subfertile, referred to fertility clinics. The evidence base is men with a diagnosed problem, not men who started trying three months ago. That does not make this a bad purchase. It means the honest expectation is nutritional insurance, not a treatment.

Two things worth knowing about what is not in it. There is no CoQ10 and no carnitine β€” and in the Cochrane analysis, carnitines and combination formulas were where the motility signal appeared, while CoQ10 is among the most-studied ingredients in this category. There is something I want to say plainly on a page for a supplement that contains tongkat ali and tribulus, because both are marketed to men as testosterone support.

Actual testosterone therapy is one of the most common preventable causes of male infertility, and most men taking it have no idea. Testosterone from outside the body suppresses the hormonal signal from the brain that tells the testes to make sperm β€” FSH and LH fall, and sperm production falls with them. A World Health Organization study of 271 men given weekly testosterone injections found that 65% had no sperm at all in their semen by six months, with the average time to that point around 120 days.

So if you are on testosterone replacement, or you have used anabolic steroids, that is very likely the biggest single factor in your fertility, and no supplement on any shelf offsets it. That is a conversation with a urologist or a fertility specialist, not a purchase. The good news is that it is often reversible once treatment stops or changes β€” but it takes months, and it needs medical supervision rather than guesswork.

Two smaller notes on this label. The vitamin D is 4,000 IU, which is exactly the tolerable upper intake level for adults β€” not a problem on its own, but it leaves no headroom if you are already taking a separate vitamin D, so check before stacking. And the very large B-vitamin percentages, B12 at 2,000% and B1 and B2 at 1,000%, look alarming and are not; these are water-soluble and routinely formulated well above the daily value.

Finally, the part that matters most and costs nothing. Stopping smoking, moderating alcohol, losing excess weight and keeping heat away from the testes have better evidence behind them than any supplement in this category. If you have been trying for twelve months β€” or six, if your partner is over 35 β€” get a semen analysis. It is quick, inexpensive, and it answers in one test what a year of guessing will not.
